PATIENT INFORMATION

Your care, step by step.

Practical information to help you prepare for an appointment and find the right support.

Before your visit

Check the hospital, clinic, date and time on your appointment letter. If you need to change your appointment, use the contact details on that letter.

Write down the questions you would like to ask and have an up-to-date list of your medicines available.

At your appointment

Your clinician will discuss your symptoms and how they affect you. They will explain any assessment or investigations you may need and talk through the next steps.

Tell the team what matters most to you, including activities you would like to return to.

After your visit

Follow the advice and information given by your care team. Keep any letters or leaflets so you can refer to your plan and the contact details provided.

If you are unsure about the plan, contact the team using the details in your appointment or discharge information.

Getting to hospital

Our department works across three hospitals. Check your letter before travelling, as your consultant’s usual base may not be your appointment location.
